About the R-U-OK Program

  • The R-U-OK Program was created to allow the Sausalito Police Department to respond effectively and efficiently to calls for service involving the elderly and/or dependent citizens living in our community
  • The program helps to inform both the Sausalito Police Department and the Southern Marin Fire Department on the location, contact, and medical information of senior citizens in our community for use in cases of disaster, power outage, or other emergency situations
  • Residents applying for the program are asked to provide a one-page form that centralizes relevant information to assist First Responders when they are called for assistance
  • The R-U-OK form asks for personal information, emergency contact information, medical information (including if the resident has a Do Not Resuscitate or “DNR” on file or not), if there is a hide-a-key on the property or with a neighbor, and if the resident has any pets
  • The R-U-OK form does not replace the use of a File of Life (Lifesaving Information for Emergencies) and should be used in conjunction with it

How to Apply for the R-U-OK Program

  • Contact the Sausalito Police Department at (415) 289-4170 to request your R-U-OK form
  • The form is made out in duplicate and the original copy should be displayed from the side of the resident’s refrigerator, the carbon copy is kept on file at the Sausalito Police Department for use only by Sausalito Police and Southern Marin Fire personnel
